2012 REGISTRATION INFORMATION

The Waverley Park Hawks are accepting registrations for the 2012 season.
The club’s next Registration Day is on Sunday 12 February 2012 at Columbia Reserve.

Re-registrations (Existing Players of Club)

In 2012 the Waverley Park Hawks will be using on-line registration for returning players.
The on-line registration process requires a username and password for use. The club will be e-mailing all registered players for 2011 who have not yet registered with the club for 2012 (to the email recorded at the time of 2011 registrations) with a user name and password to access the system in late January.
Payments using Visa or Mastercard can also be made on-line at the time of registering.
Those who wish to pay registration fees by cash or cheque, will need to first register on-line and then attend one of our Registration Days to complete payment. Alternatively, if paying by cheque then payment can be forwarded to:

New Players and Players Transferring from Another Club)

New players to the club and players transferring from another club need to 

• download and complete the SMJFL Player Registration - Transfer Form (see link at bottom of this page); and
• provide a copy of a Proof of Age document (Birth Certificate, Passport, Health Centre Book, or Baptismal Certificate)

Forms can be dropped off at one of our Registration Days with payment at our clubrooms: Columbia Reserve, Columbia Drive Wheelers Hill 3150 (Melways reference 71H11).
Alternatively, forms together with payment (cheque or Credit Card Payment Authority) and copy of a proof of age document can be sent to:
