TEAM SIGNATURE SHEET

The Team Signature sheet is a sample of your players' signatures which needs to be submitted to the league by the 4th round.
 

Step 1

Download the empty sheet from the SMJFL website on this link.

 

Step 2

Populate your team and player details. You can use the reporting functionality in SportsTG to give you the player names and DOB - see instructions below.

 

Step 3

Get your players to sign the sheet at training or a game.

 

Step 4
 

Scan and upload the sheet to the league here.
 

Note that if you have any new players join the team during the season, the signature sheet needs to be resubmitted.
 

Tip: Keep the hard copy of the signature sheet filed, so that if new players join during the season, you can handwrite their name and DOB, get the player's signature, re-scan and re-submit.
